Pre-Program Evaluation Journal

Before you start, read the following instructions:

While enrolled in the Haumana/MARC U*STAR Program, we will ask you to participate in some or all of these evaluation activities: this entering survey, post-program activity surveys, personal journal entries in response to questions, a post-program survey and a yearly personal interview.  If you have any questions, e-mail me at deeleyw001@hawaii.rr.com  Please answer ALL of the following questions!

Before you start your evaluation, make sure you disable your Internet Security software.  You must first turn off your internet browser (MS Internet Explorer or Netscape), then disable your Internet Security Software and restart your internet browser. After you disable your Internet Security software, you can answer and submit your evaluation.  Make sure you print your answers before you submit!  We had several incidents where an error occurred  in transmission and the answers were  all lost.

PLEASE ANSWER QUESTIONS THOUGHTFULLY AND IN AS MUCH DETAIL AS POSSIBLE.

For the best results, use Microsoft Internet Explorer 6.0.
